This position will include: obtaining medical records post hospital discharge, monitor HSX and hospital portals for admissions, incoming faxes, and documents, communicating with care managers in each TriValley Office, and maintain a working relationship with area hospitals and their medical record departments. Position is 40 hours per week Monday-Friday 8am-4pm.
